Pricing
Office Visits

New Patient Visit (40 minutes): $75

Established Patient Visit (20 minutes): $55

Single condition visit: $50

Monthly membership for patients who want to be seen 3 times a month (covers visit cost): $175 a month
Preventive & Wellness Care

Chronic Care & Medical Management

Chronic Complex Condition New Patient (Diabetes, HTN, etc.| 60 minutes): $110

Chronic Complex Condition Follow-Up (Diabetes, HTN, etc.| 30-40 minutes): $80

Medication Management Visit: $55

Monthly membership chronic complex conditions, for patients who want to be seen 3 times a month (covers visit cost): $220 a month
Forms & Administrative Services
